Understand the main kidney blood tests, why urine tests also matter, and when abnormal results or symptoms need prompt attention.
Your kidneys remove waste and help maintain fluid and mineral balance. A kidney function test panel, often called KFT or RFT, provides clues about these jobs. Check which tests your report actually contains, because panels are not identical everywhere.
The main results to understand
- Creatinine: A waste product from muscles that the kidneys remove. A rise can suggest reduced filtration, but muscle mass, diet, activity and some medicines also affect it. MedlinePlus: Creatinine test
- eGFR: An estimate of how well the kidneys filter blood, usually calculated using creatinine and personal details. Read it alongside previous results and other findings. NIDDK: Kidney disease tests and diagnosis
- Urea or BUN: Related measures of waste from protein breakdown. Kidney problems, dehydration and protein intake can affect the result. Check the exact test name and units before comparing reports. MedlinePlus: Blood urea nitrogen
- Electrolytes: Minerals such as sodium and potassium support nerves, muscles and fluid balance. Abnormal levels may have kidney-related or other causes. MedlinePlus: Electrolyte panel
Why might the results change?
Creatinine can rise with dehydration, urinary blockage or kidney injury. Intense exercise, muscle injury and a meat-heavy diet may also influence it. Low creatinine can reflect low muscle mass rather than unusually good kidney function. A result within the laboratory interval does not completely exclude early kidney disease. MedlinePlus: Creatinine test
Diabetes and high blood pressure are important causes of long-term kidney disease. However, one abnormal blood test does not by itself establish a chronic condition. Doctors look for persistence over time and other evidence. A new or rapidly changing result may need urgent assessment, rather than waiting for a routine repeat. NIDDK: Evaluating chronic kidney disease; NHS: Acute kidney injury
Why might you need a urine test?
A urine albumin-to-creatinine ratio, often written UACR or ACR, checks for leakage of a blood protein into urine. Blood filtration tests and urine albumin give different information, so your doctor may request both. This is especially relevant if you have diabetes or high blood pressure. Early kidney disease may cause no obvious symptoms. NIDDK: Kidney disease tests and diagnosis
What to discuss with your doctor
Bring previous results and a list of medicines and supplements, including painkillers. Mention recent vomiting, diarrhoea or difficulty passing urine. Ask whether you need repeat blood tests, urine testing or an examination. Follow the specific preparation instructions for your tests, and do not change medicines yourself. MedlinePlus: Creatinine test; NHS: Acute kidney injury
When is help urgent?
Seek urgent medical care if you pass much less urine than usual, have persistent vomiting, develop new swelling with breathlessness, or become confused or unusually drowsy. Severe breathing difficulty needs emergency care. If the laboratory or doctor reports a critical result, follow their instructions immediately, even if you feel well. NHS: Acute kidney injury
